New Patient Registration (Archived Info)
The following information describes the registration process that was previously in place. It is no longer valid.
Registration Process
The practice could only accept new patients who lived in the practice area. Registration involved completing a form which could be obtained from the practice.
Patients had the right to express which particular practitioner they wished to consult. The practice also required some form of identity, e.g. marriage or birth certificate, driving licence, paid utility bills or evidence of benefit entitlement.
When registering, patients would be asked to make an appointment to see the doctor followed by a new patient check with the practice nurse.
Patient registration would take up to 48 hours to complete.
